Premenstrual Dysphoric Disorder (PMDD) is a severe, chronic psychiatric condition, often described as a heightened sensitivity to normal hormonal changes throughout the menstrual cycle (i.e. shifts in estrogen and progesterone.) It affects about 1.2-5% of women of reproductive age. PMDD differs significantly from PMS due to the intensity of the symptoms. It involves debilitating mood, behavioral, and physical symptoms—such as severe depression, anxiety, and anger—appearing in the week before menstruation and resolving shortly after. Women may experience intense irritability, interpersonal conflict, severe depression, suicidal thoughts, hopelessness, and panic attacks.

Because PMDD involves a biological sensitivity to hormonal fluctuations rather than a simple hormonal imbalance, SSRI “antidepressant” medications are often used as the first-line treatment to provide symptom relief. These can be taken daily or intermittently (per prescriber's recommendation) to manage rapid mood shifts. In addition to pharmacology, we may collaborate with your gynecologist to use hormonal interventions. Additionally, lifestyle habits (sleep, physical activity, nutrition) are assessed to determine their impact on symptoms. Treatment is often initiated after a patient tracks symptoms for two to three menstrual cycles to differentiate PMDD from other conditions, ensuring an accurate diagnosis and a customized, often highly effective, care plan.
